1. Give back the I-94 card from your H-1B approval form(that's what I did). But if you still have concerns, simply just give back both, but remember to make copies of both cards for your record.
2. The normal place that an applicant is expected to apply for a visa is the place where you are resident and has strong work and personal ties. I would recommend you have your visa interview in Shanghai, just in case.
I just got my h1b (same situation, got h1b this october)
The visa officer took away my old F1 I-94 card, but didn't take the one came with H1b approval notice. I got a new I-94 since I took a flight entering US.
We should always keep the i94 on your h1b notice. Just surrender the white color i94 card when you go out of US airport . That is what my attorney for h1b told me .
