关于签证预约的一点疑惑

I will call from US to make appointment. I already puchased the pin on the web. Do I need to pay for the visa application fee before I can make appointment?

I currently hold H-1 visa and will apply for H1B visa in Shanghai to return to the States. Seven years ago I was refused for F1 visa and got the F1 visa at the third trial. Is this going to impact when I can get the appointment?
