i think the VO forgot to return this to you, i remember when I went for interview 2 yrs ago, the lady forgot to give it back to me and thank god i realized when i was filling out the mailing address for visa,and returned to the window asking her for my form.
i think you should call them and let them know the situation so that they can return it to you via express mail
